Going from old memories here, but IIRC, if set to yes, missed call notifications are suppressed only if one of the ringing phones is answered. If there is no answer and the call fails over to the alternate destination, all phones will show a missed call. If set to always, there is no missed call notification ever, even when a call fails over to the alternate destination.
